License Fee: $67

This license is only valid for persons working at establishments located in the Village of Sussex. This license will be valid for not less than 13 months and not more than two years.

Payments are processed through Stripe. A processing fee of 2.9% + 0.30 will be assessed by Stripe at the time of payment. The Village does not receive any revenue from these fees.

Operator licenses are processed during business hours, Monday-Friday. Village Board approval may be required.

List all tickets and violations (Federal, State and Local) including speeding and other traffic volations from age 18 to present, including pending violations and charges that were dropped. Misdemeanor, felony or traffic convictions or offenses do not necessarily exclude you from obtaining a license to serve fermented malt beverages and intoxicating liquors. Failure to list all violations may result in the rejection of this application and denial of license.

To the Village Board of the Village of Sussex, WI: I hereby apply for a license to serve Fermented Malt Beverages and Intoxicating Liquors, subject to the limitations imposed by Section 125.32(2) and 125.68(2) of the WI Statutes and all acts amendatory thereof and supplementary thereto and hereby agree to comply with all laws, resolutions, ordinances and regulations, Federal, State or Local affecting the sale of such beverages and liquors if a license is granted to me. I acknowledge that a full background check may be run by the Village through the WI Department of Justice and the DOT Public Abstract Request System. I release the Village of Sussex and all providers of information from any liability as a result of furnishing and receiving any information related to the Village of Sussex licensing process. I declare under penalty of law that I am at least 18 years of age, have completed a responsible beverage server training course in the last two years or held an operator's license in the state of WI within the last two years and that all of the above information is true and correct to the best of my knowledge and belief. I agree to electronically sign and to create a legally binding contract between the other party and myself, or the entity I am authorized to represent.
